Orthotics

Nightguards are an excellent way to protect the teeth and occlusion from night time grinding and abnormal forces. Splints and Orthotics can be used to bring the condyle into a more normal position, provide normal neuromuscular balance and relieve pain.

SOFT NIGHTGUARD #6131

Nightguard worn on the upper or the lower arch. It is made out of a soft material much like a Sports Mouthguard. It is very comfortable to wear and is good for patients that cannot tolerate hard acrylic. Only minor adjustments can be done, and it is not as durable as a hard acrylic splint.

GELB SPLINT #6203

Lower Orthotic to reposition the mandible down and forward. It is used determine the pain free position for TMJ/TMD issues. This appliance is not recommended for long term wear do to the possibility of the lower anteriors super erupting. An accurate construction bite is very important for this appliance. It should represent the desired vertical and the anterior position of the mandible.

TMJ PULL FORWARD ORTHOTIC #6201

Lower Orthotic to reposition the mandible down and forward. It is designed for long term wear. The lingual acrylic is carried up to the incisal edge of the lower anteriors to prevent super eruption. This Orthotic will bring the mandible down and forward to achieve proper spacing and function between the condyle and fossa. An accurate construction bite is very important for this appliance. It should represent the desired vertical and the anterior position of the mandible.

GERBER ORTHOTIC #6210

Dr Jay Gerber developed his version of a Neuromuscular Pull Forward Lower Orthotic. This appliance has multiple ball clasps for good retention. A lingual braided wire to strengthen the appliance. Deep indentations are placed to make sure the patient bites in the correct position. Lingual flanges prevent lateral movement. The bite for this Orthotic is usually created with the help of a TENS unit or K-7.

ANTERIOR DEPROGRAMMER #6180

This maxillary splint has a flat lingual bite plane/platform. The patient’s lower anteriors are the only teeth that touch the splint. The mandible can slide around and find a proper position and prevent wear on the teeth from grinding. There is no posterior contact with the splint.

F.A.C.T. #6203F

Dr. Terry Spahl developed the F.A.C.T. Appliance as his primary daytime Orthotic for this TMJ/pain patients. It is a modification of the Gelb Appliance. Delta clasps are used to provide the optimal retention. Buccal flanges are added to prevent lateral movement. The bite is very important for this Orthotic. Dr Spahl routinely checks the bite, appliance and condular position with a Transcranial X-Ray. A Bionator is usually worn at night to compliment the FACT during the day.

OLMOS DAYTIME ORTHOTIC: OD1

Lower Orthotic to reposition the mandible down and forward. It is used determine the pain free position for TMJ/TMD issues. It can be made with acrylic and multiple balls clasps or with a custom PMT base and acrylic added to the occlusal surface. An accurate construction bite is very important for this appliance. The phonetic “S” bite registration is a simple and effective position for this Orthotic to be made.

OLMOS DAYTIME COMPACT ORTHOTIC: OD2

The OD2 is very similar to the OD1 except for the anterior portion. The OD2 has a lingual .032 wire similar to the Gelb Appliance to connect the acrylic. The wire replaces the acrylic connector of the OD1. This appliance is not as sturdy as the OD1 and great care should be taken when inserting and removing it. It is not meant for long term wear. The same phonetic “S” bite is used for this appliance.

OLMOS NIGHT ANTERIOR DEPROGRAMMER: ON1

Upper Anterior Deprogrammer that is worn at night. It can be made with acrylic and ball clasps or the customer PMT material that eliminates the need for clasps. The flat bite plane only contacts the lower anterior teeth. There is no posterior contact. This appliance helps to increase the airway and control para-functional activity. It will help reduce clenching and grinding. An accurate construction bite is very important for this appliance. The phonetic “S” bite registration is a simple and effective position for this appliance to be made.

OLMOS NIGHT POSITIONER: ON2

This design is used for patients that lock at night when sleeping. It can be made with acrylic and ball clasps or the customer PMT material that eliminates the need for clasps. An anterior ramp is added to prevent the mandible from falling back at night. The ramp engages the lower anteriors to hold the mandible forward. There is no posterior contact. An accurate construction bite is very important for this appliance. It may be necessary to open the vertical more than the Daytime Appliance in order to accommodate the ramp.
